Free diabetes screenings, education, and lifestyle support designed for older adults — delivered at senior centers, adult day programs, and assisted-living facilities so attending is easy and familiar.
What we provide
- A1C and blood-pressure screening on site
- One-on-one consultation with a physician and registered nutritionist
- Age-appropriate prevention and management education
- Discussion of medication interactions and management
- Large-print materials in English, Urdu, Hindi, and Spanish
- Coordination with on-site nursing or care staff
