Health is hyperlocal. So are we.
Diabetes prevention works when it meets people in places they already trust. We bring our physicians, nutritionists, equipment, and education to faith communities, civic organizations, and local clinics across the Chicagoland area.
What we bring
A complete screening event — at no cost to your community
- A registered nutritionist and a physician on site
- A1C testing equipment and blood-pressure cuffs
- Printed prevention materials in English, Urdu, Hindi, and Spanish
- One-on-one consultation time for every attendee
- Follow-up plan and referral information
- Light, healthy refreshments
What you bring
Just the room and your community
- A room with 6–8 tables, ideally with chairs for a small consultation area
- Help promoting the event to your members (we provide flyers and graphics)
- An on-site contact to greet attendees
- 2–3 hours on the day of the event
